understand that dog training is not without risk to myself,
my dog, members of my family, or guests who might attend
my training session. I hereby release “Canine Action”,
its employees, volunteers, officers, and agents from any
and all liability of any nature, for injury or damage which
I or my dog may experience during training sessions while
on the training grounds and surrounding property. I understand
that I must type my name in this box in order to accept
this liability waiver prior to attending any classes. My
typewritten name will serve as my electronic signature. |

Things
to bring the first night of class:
1.
Your dog with a flat, buckle collar and six foot leash.
No choke collars, prong collars, extension leashes, or chain
leashes.
2. Semi-moist treats that your dog loves and that are only
used in training.
3. Good attitude.
You may pay by cash, check, or credit card on the first
night of class. If you wish, you can mail your payment,
by check or credit card, to the address at the top of the
form. If you are mailing your credit card number, please
include the expiration date. You may also make a payment
directly at G.O.D.O.G.S. in person or over the phone. You
can also pay by credit card now while registering. Follow
the instructions after submitting this form. |
