Consult
Format
1. Contact
us by phone or email to schedule the initial interview.
It
is important for us to have as much information as
possible about the pet, it's environment, and the
problems you are experiencing before the initial behavior
consult. This helps us determine where we need to
focus our attention and what further questions we
need to ask. To do this, we ask that you fill out
our 5 page behavioral history form before the initial
consult. You can fill it out in Word and forward it
via email as an attachment or mail it to the address
listed on the "contact us" page. Click on

2. Comprehensive
in-home behavioral consult
3. Formulation
of behavior modification plan
4. Follow-up
in-home visit to go over modification plan, demonstrate
behavior modification techniques, and answer any questions
you might have about the plan.
